Credential Files - Registration
Dear Registrant:
The Credential File consisting of your letters of reference is a required part of many school districts' application process. In addition, registration with the Human Sciences Career Services Office enables us to refer your name to school distrticts seeking specific types of candidates (Spanish, middle school, special education, etc.)
Linked below are the forms necessary to open your cendential file with Human Sciences Career Services.
[Special Note: Transcripts are NOT included as a part of your Cedential File. An official copy should be requested separately through the Registrar's office at 515/294-1840. Copies of an official transcript are generally acceptable to school districts.]

- A nameplate is the first document that a hiring official sees upon opening your credential file. Please follow the link for an example.
- The Sample Reference Letter is for you to copy and distribute, giving permission to anyone writing a recommendation for your file. Read through it carefully, fill out the top section and then provide it to your references with a copy of your resume and a pre-addressed envelope.
- The Service Information Sheet helps you to understand how to request your crednetial file be sent and indicates costs for our services. This sheet is yours to keep for future reference. Payment can be made via check or Visa, Master Card or Discover.
